In porn, this means a situation or setup that is created to give the appearance of a non-scripted or non-planned situaiton.

Reality Porn revolves mostly around the street pickup or the interview setup to give the impression of people who were not aware that they would be part of a porn movie. This is very popular in Japan, where the street pickups are often quite real. In the US, sites like Bang Bus and MILF Hunter use this setup to great effect, while others use it quite poorly and make it very obvious that it is a setup.
